Social Worker Case Manager
Location: Cambridge MA 02138
Rate: $42-45 per hour

The Ambulatory Case Manager provides case management services to patients in an outpatient healthcare setting. This role focuses on assessing patient needs, coordinating care, monitoring healthcare utilization, and developing individualized care plans to support quality outcomes and continuity of care. The Case Manager works closely with physicians and other clinical staff to address patient concerns, identify opportunities for improvement, and ensure appropriate use of healthcare resources.

• Provide ambulatory case management services through patient interviews, medical record reviews, research, and assessment of healthcare needs.
• Develop, implement, and monitor individualized care plans based on patients' medical and psychosocial needs.
• Collaborate with physicians and clinical staff to coordinate comprehensive outpatient care and ensure continuity of services.
• Communicate regularly with treating clinicians regarding patient concerns, care coordination needs, and clinical updates.
• Monitor healthcare utilization patterns and identify opportunities to improve care delivery and resource utilization.
• Promote the effective application of case management principles and best practices across clinical services.
• Educate medical staff on appropriate case management criteria, clinical guidelines, and utilization standards.
• Work with team leadership to identify concerns in clinical practice patterns and assist in developing solutions.
• Identify potential quality-of-care issues and collaborate with clinical teams to address concerns.
• Assist in developing reports, resources, and documentation to support case management activities and departmental needs.
• Support continuous improvement initiatives focused on patient care, care coordination, and operational efficiency.

• Minimum of three years of social work experience in a medical or healthcare setting.
• Knowledge of managed care and case management principles.
• Familiarity with the care needs of medically complex pediatric, adult, and geriatric populations.
• Strong organizational, analytical, and problem-solving skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with multidisciplinary clinical teams and manage competing priorities.

• Experience in ambulatory or outpatient case management.
• Experience coordinating care for patients with complex medical conditions.
• Familiarity with healthcare utilization review, care planning, and quality improvement initiatives.
• Experience working with physicians and multidisciplinary healthcare teams.
